Requesting a consultation?
Initial consultations are available to prospective patients. Select "Consultation Request" as your subject and include any relevant medical background or questions in your message.
A valid Maine MMMP patient card and a valid state-issued photo ID are required to become a registered patient. If you don't have a patient card yet, your caregiver can point you toward the right resources.
